Cancelation Policy
If you need to cancel your appointment less than 48 hours before the scheduled start time, a $50 late cancellation fee will be charged.

You’ll receive a secure video link in your appointment confirmation email, and again by text on the morning of your visit. Please sign on 5 minutes early so that we can maximize your appointment time.

You can request a refill through your pharmacy or send a message in your Elation Patient Portal.

Once your lab results are finalized, you can view them in your Elation Patient Portal. If you have any questions, just send us a message and we’ll be happy to help!
