Interested in converting from By-Appointment Care to our Primary Care Membership? You have the option to backdate your membership to your last appointment and apply that appointment charge to your annual membership fee.
Primary Care Membership Charges
✓ For the first year, the membership fee is billed in advance at an annual rate of $1,800.
✓ After the first year, the membership renews at $150/month on a month-to-month basis.
Cancelation and Reschedule Fees
We welcome patients to reschedule or cancel their appointments through their appointment reminder email or by calling the office—there’s no fee to do so. The only exception is late changes: rescheduling within 24 hours of your appointment or cancelling within 48 hours of your appointment will incur a $50 fee. Because we cannot reliably fill appointments with short notice, this policy protects our physicians’ limited availability and ensures appointment times remain open for patients who need them.
Accessing Our Mental Health & Pelvic Health Network
We partner with a trusted network of mental health and pelvic health providers to support your care. These services are not billed through Curate—each provider manages their own fees and policies.
Our curated listings are offered as a courtesy and added resource for our patients and care community. While we do our best to share accurate pricing information, final details about costs, insurance, and billing will always come directly from the individual practice.
