Membership plan details priced at $150 per month, offering primary and preventive care, menopause care, metabolic and weight care, breast cancer supportive care, chronic condition management, prescriptions and lab orders, and care coordination with specialist referrals. Interested in converting from By-Appointment Care to our Primary Care Membership? You have the option to backdate your membership to your last appointment and apply that appointment charge to your annual membership fee.

Primary Care Membership Charges

✓ For the first year, the membership fee is billed in advance at an annual rate of $1,800.

✓ After the first year, the membership renews at $150/month on a month-to-month basis.
